Created by Dale Mahalko, using Microsoft Word -- mostly to demonstrate what can be done, since Word was never intended for this purpose, and because I don't have a formal drawing program available. Page zoom set to 200% in Word, then screenshotted and saved as a JPEG using Corel PhotoPaint.
